Long-Term Corticosteroid Treatment May Negatively Affect Overall Health of Patients With Chronic Inflammatory Demyelinating Polyradiculoneuropathy: Presented at EFNS

GENEVA -- September 29, 2010 -- Patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) who receive long-term corticosteroid treatment may have an increased risk for various side effects compared with those receiving a short-term corticosteroid or intravenous immunoglobulin (IVIG) therapy, according to a study presented here at the 14th Congress of the European Federation of Neurological Societies (EFNS).

Deborah A. Lee, MD, PhD, Baxter BioScience, Westlake Village, California, reported on September 26 that, because different options are available for the treatment of CIDP and negative side effects have been reported with long-term corticosteroid use, researchers compared the risks associated with long-term versus short-term corticosteroid or IVIG treatment.

The researchers analysed the MarketScan database to determine the risk of developing specific side effects associated with 3 treatment modalities in patients diagnosed with CIDP. Patients were followed for 3 years and received long-term (n = 84) or short-term (n = 156) corticosteroid therapy or IVIG (n = 177). Data were evaluated by chi-square and nonparametric tests to determine the number and rate of specific side effects within the groups. Risk was determined by multivariate analysis.

A significant (P =.001) association was found with 4 preidentified corticosteroid side effects in long-term versus short-term use: diabetes (odds ratio [OR] 2.37), heart failure (OR 2.34), myopathy (OR 5.82), and fractures (OR 2.56). Patients in the long-term corticosteroid arm had a significantly (P <.0001) higher risk of developing hypernatraemia, diabetes, heart failure, myopathy, anxiety, and osteoporosis (OR 1.97, 2.97, 3.65, 4.2, 4.95, and 2.11, respectively) than patients in the IVIG arm.

The researchers concluded that the long-term use of corticosteroids may negatively affect patients with CIDP and increases the risk of several side effects previously associated with corticosteroid use. As a result, careful consideration should be given when placing patients with CIDP on long-term corticosteroid therapy.

Interrupted long-term corticosteroid therapy was an issue that was complicated by symptom reappearance, Dr. Lee added.
